What Family Therapy Actually Is

Sessions with several family members present, working on the patterns between them rather than on any single person's symptoms. The subject is the interaction — who speaks, who is spoken over, what cannot be said, and what each person does when the tension rises.

Not everyone attends every session. It is common to work in different combinations, and often to see individuals separately at points.

Repeating conflict that never resolves. Communication that has broken down between generations. Adjusting after divorce, remarriage or a blended household. Supporting a family through the illness or dependency of one member. Shared grief after a bereavement, where each person is grieving differently and reading the others as uncaring.

It is also useful where one person has become the identified problem. Often that person is expressing something the family cannot discuss directly, and treating them alone changes little.

Where there is abuse or coercive control, joint sessions are not appropriate and can increase risk. If this describes your household, seek individual support and contact a domestic violence service. Safety comes first.

It is also unsuitable when one member is in acute crisis and needs individual or psychiatric care first, when someone is being brought under duress and has no wish to participate, or when a family arrives expecting the therapist to adjudicate and declare someone at fault.

Not a tribunal. The therapist will not rule on who is right, and a family hoping for that verdict usually leaves disappointed.

Not a way to fix one person while everyone else observes. If the family's position is that one member is the problem and the rest are fine, that assumption will itself become the subject.

Households often extend well beyond the couple and children, and the boundary questions are different — in-law involvement in a marriage, obligations to parents, decisions taken collectively, expectations around career and marriage.

Approaches developed for nuclear families do not transfer directly. A therapist who treats extended family involvement as inherently unhealthy has misread the context; the useful work is usually about boundaries and communication rather than separation.

Will old conflicts be dragged up?+

Only where they are still operating in the present. The focus is on what is happening now, though present patterns often have long histories that need acknowledging.

Is what we say confidential?+

Yes, outside the family. Within it, confidentiality is more complex — how information shared in individual sessions is handled should be agreed explicitly at the start rather than assumed.
