Psychologist, Psychiatrist or Counsellor: What Is the Difference?

The titles overlap, the training differs, and in India the words are used loosely enough that a great many people book the wrong appointment. This is a plain guide to who does what.

Understanding Your Healing Journey

Treatment & Support

Psychiatrist

Psychiatrist

A medical doctor who has specialised in mental health, holding an MBBS followed by psychiatric training. Because they are doctors, they can prescribe medication and order medical investigations.

See a psychiatrist if you may need medication, if symptoms are severe or disabling, if there are thoughts of suicide or self-harm, or if a physical cause needs ruling out. Most psychiatrists do not provide extended talking therapy — they typically manage medication and refer you to a therapist for the rest.

Clinical Psychologist

Clinical Psychologist

Holds a postgraduate degree in psychology plus specialised clinical training, usually an MPhil in Clinical Psychology in India. They diagnose and treat mental health conditions and carry out formal psychological assessment and testing.

They cannot prescribe medication. They tend to work with more severe or complex presentations, and frequently alongside a psychiatrist.

Counselling Psychologist

Counselling Psychologist

Holds a postgraduate degree in psychology or counselling psychology. Works therapeutically with emotional difficulties, life problems, relationships and personal development.

In practice the overlap with clinical psychology is substantial. The rough distinction is that counselling psychology leans towards life difficulties and personal growth, and clinical psychology towards diagnosable disorder — but both treat anxiety and depression routinely.

Counsellor or Therapist

Counsellor or Therapist

Here it becomes difficult. Neither title is legally protected in India, so qualifications behind them vary enormously — from a full postgraduate degree to a short certificate course.

This is not an argument that every counsellor is underqualified. Many are excellent. It is an argument for asking directly what someone's training is, because the title alone tells you very little.

Psychotherapist

Psychotherapist

Someone trained in a specific therapeutic method — psychodynamic therapy, CBT, and others. It describes the training rather than a protected professional status, and is often held alongside one of the qualifications above.

So Which Do You Need?

So Which Do You Need?

If you might need medication, or symptoms are severe: start with a psychiatrist. If you want to work through anxiety, low mood, grief, relationships or a life difficulty: a counselling psychologist. If you need formal assessment or have a complex diagnosis: a clinical psychologist.

If you are unsure, start with a counselling psychologist. A competent one will refer you onward if you need something they cannot provide, and that referral is part of doing the job properly.

Questions & Answers

Everything you need to know before booking your first session.

Can a psychologist prescribe medication in India?+

No. Only a medical doctor, in practice a psychiatrist, can prescribe psychiatric medication. Psychologists of any description cannot.

Do I need a psychiatrist and a psychologist?+

Sometimes, and the combination is common. Medication can reduce symptoms enough for therapy to be usable, while therapy addresses the underlying pattern. Many people see both.

Is seeing a psychiatrist a sign things are serious?+

Not necessarily. People consult psychiatrists for moderate anxiety and sleep problems too. The relevant question is whether medication might help, not how bad things have become.

How do I check someone's qualifications?+

Ask which degree they hold and from where, and which body certified any additional training. A qualified practitioner will answer without hesitation. Reluctance to answer is itself an answer.
