The Basic Idea

Thoughts, feelings and behaviour are connected, and the connection runs in every direction. Change one deliberately and the others move.

The insight that makes CBT useful is that a great deal of distress comes not from events but from the interpretation attached to them, and that these interpretations are frequently habits rather than assessments.

Someone does not reply to your message. Interpretation A: they are busy. Interpretation B: they are annoyed with me and I have probably ruined it. Same event, entirely different afternoon.

CBT does not claim interpretation B is never right. It claims that if you reliably reach for B, that is worth examining — because you are treating a habit as evidence.

What Actually Happens in Sessions

Considerably more structure than most therapy.

You identify specific situations and what ran through your mind in them. You learn to notice the patterns your particular mind favours — catastrophising, mind-reading, all-or-nothing thinking, taking responsibility for things well outside your control.

Then, crucially, you test them. Not by insisting on a positive alternative, but by asking what the actual evidence is, and frequently by running an experiment. Someone convinced they will be visibly humiliated if they speak up in a meeting is asked to speak up in a meeting and find out.

There is homework. This is not incidental — the between-session work is where most of the change happens, and people who do not do it get considerably less from it.

Sessions are usually time-limited: often eight to twenty, with a defined focus.

What the Evidence Says

CBT has the largest evidence base of any psychotherapy, and it is genuinely strong for anxiety disorders, panic, phobias, OCD, depression, insomnia and PTSD. For several of those it performs comparably to medication, and combined treatment often outperforms either alone.

Two caveats worth knowing. Its evidence base is partly large because CBT is structured and manualised and therefore easy to study — comparatively, not because other therapies have been shown to fail. And effect sizes in real clinics are typically more modest than in trials, where participants are screened and therapists are supervised closely.

It is very good. It is not the only thing that works.

Where It Is Oversold

Where the difficulty is not a distorted thought but an accurate one. If you are in a genuinely bad marriage or an exploitative job, examining the evidence for your unhappiness confirms it. The problem is the situation, and no amount of thought-challenging fixes it.

Where distress is held in the body rather than in cognition. Trauma responses frequently do not yield to reasoning, because the part of the system generating them is not reachable by argument. Somatic approaches and EMDR-type work often do better.

Where the pattern is old, relational and largely outside awareness. Six sessions of structured worksheets rarely shifts something laid down in childhood.

And where a person simply does not think in that register. Some people find it mechanical, and disliking your therapy is a decent predictor of not benefiting from it.

How It Is Used Here

Not as a doctrine. CBT techniques are used where they fit — anxiety, panic, phobias, OCD, insomnia — because for those they are the most efficient route to change, and often produce something usable in the first fortnight.

Where the difficulty is somatic, relational or trauma-based, they are combined with somatic work, inner-child work, EFT and clinical hypnotherapy, which reach material that argument does not.

A practitioner offering exactly one method to everyone who walks in is describing their training, not your problem.

How long does CBT take?+

Commonly eight to twenty sessions for a defined problem. Simple phobias can resolve in a handful; long-standing depression takes longer.

Is CBT just positive thinking?+

No, and it explicitly is not. You are testing thoughts against evidence, not replacing them with cheerful ones. Where the pessimistic thought turns out to be accurate, that is the finding and the work moves to what can be done about it.

Does it work online?+

Yes. CBT is among the best-evidenced therapies for remote delivery, partly because its structure and worksheets translate cleanly to video.

What if I have tried CBT and it did not help?+

Common, and it is information rather than failure. It often means the difficulty is not primarily cognitive, or the fit with the therapist was wrong. Both are worth discussing rather than concluding that therapy does not work for you.
