What Gets in the Way

Not, mostly, a belief that therapy is useless. Something more specific.

A man's standing in most Indian families rests on being the one who copes. He is the provider, the one relatives call in a crisis, the one who does not fall apart. Admitting to struggling is not experienced as vulnerability but as a failure at the role itself.

There is also, straightforwardly, no practice at it. Boys are trained out of describing internal states early — bravery is praised, distress is redirected — and by thirty a man may genuinely lack the vocabulary. Not evasion: an absence of a skill that was never taught.

Then the practical version. Therapy occurs on weekdays, and taking time off invites the question of why, in a workplace where a mental health reason is not a safe answer.

And the reasonable fear that opening it is dangerous — that if the coping stops, everything held together for fifteen years comes apart.

Depression in Men Frequently Does Not Look Like Sadness

This is the part that matters most, because it explains why it goes unrecognised by families and by men themselves.

A man presenting like this is described by his family as having a short temper or drinking too much. Nobody says depressed, including him.

Which is how men reach crisis without a single intervening conversation.

  • «Irritability and anger, often the first sign and read by everyone as temper
  • «Withdrawal into work, or into the phone, or into anything absorbing
  • «Drinking more, and later, and increasingly alone
  • «Physical complaints — back pain, stomach trouble, headaches — investigated repeatedly with nothing found
  • «Recklessness with money, driving, gambling
  • «Total loss of interest in sex, which is frequently interpreted as a relationship problem
  • «Numbness rather than pain, described as feeling flat or switched off

The Anger

Worth taking on its own, because it is the most misread symptom in the whole picture.

A man snapping at his wife and children, apologising, meaning it, and doing it again is usually not describing a temper problem. He is describing a threshold that has dropped, and thresholds drop under depression, chronic stress and sustained sleeplessness.

Anger management taught as technique alone has limited effect here, because the anger is downstream. Treat what is producing it and the temper usually resolves with it.

Meanwhile the cost accumulates: children who are wary of him, a marriage worn thin, and a growing private conviction that he is a bad man — which deepens the depression that generated the anger.

What Gets Men Through the Door

Rarely a decision to work on themselves. Usually one of four things.

An ultimatum from a wife, which is the single most common route.

A physical symptom that turned out to have no physical cause, after which a doctor suggested it might be stress.

Something concrete and bounded — insomnia, quitting smoking, performance anxiety, a phobia. Men come far more readily for a specific problem than for a general state, and a substantial amount of good work starts as something narrow.

Or a crisis, which is the worst route and the most common after the first.

What Actually Works

Framing it as a task. Many men engage well with therapy presented as a problem to be worked on, with a structure and a goal, and less well with open-ended exploration.

Starting with the concrete. Sleep, anger, a phobia. Once something measurable improves, the rest usually becomes discussable.

Not requiring emotional fluency on day one. It is entirely acceptable to arrive unable to name what you feel; developing that is part of the work rather than a prerequisite.

Clinical hypnotherapy and somatic work, which several men find considerably easier than talking, because they do not depend on articulating something you cannot yet articulate.

Evenings, weekends and online sessions, which remove the leave-from-work problem entirely.
